ABSTRACT

Our recent studies for nonnucleoside reverse transcriptase inhibitors identified a highly potent compound JK-4b against WT HIV-1 (EC50 = 1.0 nmol/L), but the poor metabolic stability in human liver microsomes (t 1/2 = 14.6 min) and insufficient selectivity (SI = 2059) with high cytotoxicity (CC50 = 2.08 μmol/L) remained major issues associated with JK-4b. The present efforts were devoted to the introduction of fluorine into the biphenyl ring of JK-4b, leading to the discovery of a novel series of fluorine-substituted NH2-biphenyl-diarylpyrimidines with noticeable inhibitory activity toward WT HIV-1 strain (EC50 = 1.8-349 nmol/L). The best compound 5t in this collection (EC50 = 1.8 nmol/L, CC50 = 117 μmol/L) was 32-fold in selectivity (SI = 66,443) compared to JK-4b and showed remarkable potency toward clinically multiple mutant strains, such as L100I, K103N, E138K, and Y181C. The metabolic stability of 5t was also significantly improved (t 1/2 = 74.52 min), approximately 5-fold higher than JK-4b in human liver microsomes (t 1/2 = 14.6 min). Also, 5t possessed good stability in both human and monkey plasma. No significant in vitro inhibition effect toward CYP enzyme and hERG was observed. The single-dose acute toxicity test did not induce mice death or obvious pathological damage. These findings pave the way for further development of 5t as a drug candidate.

ABSTRACT

Long noncoding RNAs (lncRNAs) play a critical role in the regulation of atherosclerosis. Here, we investigated the role of the lncRNA growth arrest-specific 5 (lncR-GAS5) in atherogenesis. We found that the enforced expression of lncR-GAS5 contributed to the development of atherosclerosis, which presented as increased plaque size and reduced collagen content. Moreover, impaired autophagy was observed, as shown by a decreased LC3II/LC3I protein ratio and an elevated P62 level in lncR-GAS5-overexpressing human aortic endothelial cells. By contrast, lncR-GAS5 knockdown promoted autophagy. Moreover, serine/arginine-rich splicing factor 10 (SRSF10) knockdown increased the LC3II/LC3I ratio and decreased the P62 level, thus enhancing the formation of autophagic vacuoles, autolysosomes, and autophagosomes. Mechanistically, lncR-GAS5 regulated the downstream splicing factor SRSF10 to impair autophagy in the endothelium, which was reversed by the knockdown of SRSF10. Further results revealed that overexpression of the lncR-GAS5-targeted gene miR-193-5p promoted autophagy and autophagic vacuole accumulation by repressing its direct target gene, SRSF10. Notably, miR-193-5p overexpression decreased plaque size and increased collagen content. Altogether, these findings demonstrate that lncR-GAS5 partially contributes to atherogenesis and plaque instability by impairing endothelial autophagy. In conclusion, lncR-GAS5 overexpression arrested endothelial autophagy through the miR-193-5p/SRSF10 signaling pathway. Thus, miR-193-5p/SRSF10 may serve as a novel treatment target for atherosclerosis.

ABSTRACT

The central venous access device (CVAD) is widely used in infusion of intravenous high nutrient solution, chemotherapy drugs, rapid infusion, blood transfusion, etc. CVAD is left in the patient′s blood vessels for a long time, although it has certain advantages in intravenous therapy, but also risks associated with it. In the past 20 years, many studies have focused on the mechanism of CVAD on vascular injury, trying to explore its mechanism from a different perspective at the microscopic level of pathophysiology. This article will review the three aspects of acute damage to the blood vessel caused by CVAD catheterization and chronic changes caused by indwelling catheters, analysis of the causes of vascular injury caused by CVAD catheterization, and research on preventing vascular injury caused by CVAD catheterization, aiming at providing reference for clinical intravenous therapy.

ABSTRACT

Bicyclol is a synthetic drug for hepatoprotection in clinic since 2004. Preliminary clinical observations suggest that bicyclol might be active against hepatitis C virus (HCV) with unknown mechanism. Here, we showed that bicyclol significantly inhibited HCV replication and in hepatitis C patients. Using bicyclol as a probe, we identified glycolipid transfer protein (GLTP) to be a novel restrictive factor for HCV replication. The GLTP preferentially bound host vesicle-associated membrane protein-associated protein-A (VAP-A) in competition with the HCV NS5A, causing an interruption of the complex formation between VAP-A and HCV NS5A. As the formation of VAP-A/NS5A complex is essential for viral RNA replication, up-regulation of GLTP by bicyclol reduced the level of VAP-A/NS5A complex and thus inhibited HCV replication. Bicyclol also exhibited an inhibition on HCV variants resistant to direct-acting antiviral agents (DAAs) with an efficacy identical to that on wild type HCV. In combination with bicyclol, DAAs inhibited HCV replication in a synergistic fashion. GLTP appears to be a newly discovered host restrictive factor for HCV replication, Up-regulation of GLTP causes spontaneous restriction of HCV replication.

ABSTRACT

Objective@#To study the response characteristics of the posterior intralaminar nucleus (PIN) of auditory thalamus in VGluT2-Cre transgenic adult mice when exposed to white noise and 10K pure tone stimulation.@*Methods@#All adult male Vglut2-Cre mice (8-12 weeks) were used in this study between Oct, 2017 and Oct, 2018. Using the calcium signal fiber photometry method, optic fiber was employed to locate on PIN by injecting AAV-hSyn-DIO-GCaMP6m virus, and thereafter, the activity of the target cluster neurons during different acoustic stimuli was recorded. Matlab was used for data processing and statistical analysis.@*Results@#(1)In both white noise and 10 kHz pure tone as a continuous three-second stimulation, the peak amplitude of calcium signal activity generated in PIN by white noise was superior to that of pure tone, the statistic result showed significantly difference (n=6, t=2.404, P=0.037 1) . (2)In addition, when white noise and 10K pure tone played as consecutive 3 or 5 pips within three-second stimulation, the stimulus-following ability in a consecutive 3 pulses play within 3 seconds was far better than a consecutive 5 pips play within 3 seconds (in both white noise and 10 kHz pure tone), yet consecutive 3 pips play showed greater signal attenuation speed than that in consecutive 5 pips play, the statistic result showed significantly difference (n=6, t=2.748 P=0.033 4) .(3)Regardless of the intra-group comparisons between white noise and 10 kHz pure tone stimulation, PIN showed better signal response in a consecutive 3 pips play than consecutive 5 pips play or a continuous three-second stimulation. When came to the statistical analysis, the acoustic response degree of a continuous three-second stimulation was an intermediate between two others, both consecutive 3 or 5 pips play showed significantly difference.@*Conclusions@#The results suggest that under the same acoustic intensity, VGluT2-Cre transgenic adult mice′s PIN shows greater signal response in white noise than pure tone. PIN shows greater signal attenuation to repetition play of 10 kHz pure tone, which implies PIN shows stronger adaptation to 10 kHz pure tone than to white noise. Lastly, PIN is more responsive to a complex sound information (white noise) than to simple sound information (pure tone).

ABSTRACT

Objective To investigate clinical efficacy of N-acetylcysteine (NAC) in the adjuvant treatment of patients with acute exacerbation of asthma-chronic obstructive pulmonary disease overlap (ACO). Methods One hundred and forty-six patients with acute exacerbation of ACO admitted to department of respiratory and critical care medicine of Henan provincial people′s hospital from March 2016 to August 2018 were divided into control group and observation group according to random number table, with 73 patients in each group. The control group was given clinical treatment such as bronchodilator, inhaled glucocorticoid and low- flow oxygen inhalation during hospitalization, and inhalation of budesonide/formoterol fumarate powder combined with tiotropium was continued after discharge. The treatment lasted for 4 weeks. The observation group was added with NAC 600 mg, twice daily, totally for 4 weeks on the basis of the control group. Baseline data and duration of hospital stay were collected. Before and after treatment, blood routine, c-reactive protein (CRP), forced expiratory volume in one second (FEV1)/forced vital capacity (FVC) and FEV1% pred were recorded. Clinical efficacy and adverse drug reaction were compared between the two groups. Results The white blood cell count, eosinophil count, neutrophil count, CRP, FEV1/FVC and FEV1% pred of two groups were significantly ameliorative after treatment compared with those before treatment, and the difference was statistically significant (P < 0.05). The white blood cell count, eosinophil count, neutrophil count and CRP after treatment in observation group were significantly lower than those in control group: (7.40 ± 1.12)×109/L vs. (8.34 ± 1.56) ×109/L, (0.30 ± 0.15) ×109/L vs. (0.42 ± 0.18) ×109/L, (3.76 ± 1.49) ×109/L vs. (4.58 ± 1.72) × 109/L, (5.33 ± 1.65) mg/L vs. (5.95 ± 1.74) mg/L, and the differences were statistically significant (P<0.05). There were no statistical differences in FEV1/FVC and FEV1%pred after treatment between observation group and control group (P > 0.05). The total effective rate after treatment in observation group was significantly higher than that in control group: 94.52% (69/73) vs. 82.19% (60/73), and the difference was statistically significant (P<0.05). During the treatment, duration of hospital stay in observation group was shorter than that in control group: (5.82 ± 2.29) d vs. (7.25 ± 3.05) d, and the difference was statistically significant (P<0.05). The incidence of adverse reactions in observation group was compared with control group: 10.96% (8/73) vs.5.48% (4/73), and the difference was not statistically significant (P > 0.05). Conclusions NAC has a significant effect in adjuvant treatment of acute exacerbation of ACO, which can significantly inhibit the level of inflammation and shorten the length of hospital stay.

ABSTRACT

Objective To understand the vitamin D status and its possible ethnic differences among Uygur and Han women of childbearing age in a community of Urumqi.Methods Based on the data of 2 080 patients with thyroid disease in a community of Urumqi,which were collected in May 2013,we selected the complete information of 652 Uygur and Han women of childbearing age (between 18-45 years),which included 367 Uygur women [average age (35.14± 6.91) years] and 285 Han women [average age (35.52± 7.10) years].The basic data of height,weight,and waist circumference were recorded.The serum level of 25-hydroxyl vitamin D [25(OH) D] was detected by chemiluminescence method.Results The serum 25(OH) D levels of Uygur and Han were (11.51±6.31) μg/L and (16.43±6.43) μg/L,respectively.The 25(OH)D level in Uygur women was significantly lower than that of Han women (t =-9.800,P<0.001).The constituent ratio of vitamin D status was significantly different between Uygur and Han women (vitamin D severe deficiency:47.4% in Uygur women and 12.3% in Han women),deficiency (44.7% in Uygur women and 64.6% in Han women),insufficiency 5.7% in Uygur women and 17.9% in Han women),sufficiency (2.2% in Uygur women and 5.2% in Han women) (x2 =99.486,P<0.001).Conclusion Serum 25 (OH) D deficiency is common in Uygur and Han women of childbearing age in Urumqi,especially among Uygur women.

ABSTRACT

Purpose To investigate the clinicopathological characteristics,diagnosis and differential diagnosis of benign metastasizing leiomyoma (BML).Methods The clinicopathological data in 6 patients with BML were collected.All cases of BLM were investigated by HE and immunohistochemistry of EnVision method.Results All cases were female,with age of 33 -65 years,and had undergone myomectomy.5 cases had lung metastasis,including abdominal wall metastasis and spinal metastasis in each of the 1 cases,and another case had inguinal metastasis.Morphology showed that the tumor cells were spindle without obvious atypia,nuclear mitoses and necrosis,some cases were cellular.Immunohistochemical staining showed that the tumor cells were positive for SMA,SM-MHC,desmin,ER,PR,vimentin,while negative for S-100,CD117,CD34.Ki-67 label index were less than 5%.3 patients were alive with tumor and 3 patients were alive without tumor in the follow up of 18,28,40,31,36,80 months.Conclusion BML often occurs in female patients that undergone uterine myomectomy.The lung is the most common site of metastasis,often accompanied by other sites.The disease progresses slowly,and most patients have a longer survival time.

ABSTRACT

Pyriform sinus fistula is a rare clinical disease, often with recurrent acute suppurative thyroiditis or neck infection in children.The traditional method treatment is complete resection of the fistula with or without hemithyroidectomy in external cervical surgical approach, but in recent years, minimally invasive endoscopic occlusion of the internal opening for the treatment of pyriform sinus fistula is performed in many hospitals, including electrocautery, chemocautery, laser cauterization, biocauterization, and suture closure. Literatures about endoscopic management of pyriform sinus fistula in children are reviewed and various surgical methods, complications and success rate are evaluated.

ABSTRACT

Objective To investigate the effect of aspirin combined with atorvastatin in the treatment of diabetic atherosclerosis.Methods 120 patients with diabetic atherosclerosis form August 2014 to February 2016 were selected,and randomly divided into treatment group and control group,60 cases in each group.Control group treated with aspirin enteric-coated tablets,the treatment group treated with aspirin enteric-coated tablets combined with atorvastatin,comparing the two groups before and after treatment of fasting blood glucose,blood lipid related index,carotid intima-media thickness and plaque thickness,and the adverse reactions.Results After treatment, the levels of fasting blood glucose and fasting insulin in treatment group were significantly lower than control group(P<0.05),the carotid intima-media thickness and plaque thickness in treatment group was significantly lower than control group(P<0.05).The levels of TG,TC,LDL-C and HDL-C in treatment group were significantly improved(P <0.05).Conclusion Aspirin enteric coated tablets combined with atorvastatin efficacy significantly in the treatment of diabetic atherosclerosis.

ABSTRACT

Objective To investigate the risk factors of cerebral infarction in elderly patients with type 2 diabetes mellitus.Methods Elderly patients with type 2 diabetes complicated with cerebral infarction from August 2014 to March 2016 in our hospital were selected as observation group of 70 cases, elderly patients with type 2 diabetes alone of 70 cases as control group.The related indicators were compared between two groups, then the indicators with statistical difference were included into the multiple Logistic aggression analysis.Results There were significant difference between two groups of waist to hip ratio (WHR), body mass index (BMI), fasting blood glucose (FBG), 2h postprandial blood glucose (2h PBG), fasting insulin (FINS), glycosylated hemoglobin (HbA1C), triglyceride (TG) and low density lipoprotein cholesterol (LDL-C).The multiple Logistic aggression analysis showed BMI,TG,LDL-C,FINS,2hPBG and WHR were risk factors in elderly type 2 diabetes mellitus complicated with cerebral infarction. Conclusion There are many risk factors in elderly patients with type 2 diabetes complicated with cerebral infarction, higher blood sugar, higher blood lipid and higher body mass index as the independent risk factors, which should be payed attention to and prevent the occurrence of cerebral infarction.

ABSTRACT

OBJECTIVES: We sought to identify the expression of CD68-tumor-associated macrophages (TAMs) and CD34-microvascular density (MVD) in laryngeal squamous cell carcinoma (LSCC), to study the relationship with clinical pathological parameters and to determine whether their expression is predictive of disease. METHODS: Pathologically confirmed 45 LSCC tissue and 20 peritumoral non-tumor tissue were examined. Immunohistochemical studies were used to detect the expression of CD68-TAMs and CD34-MVD. RESULTS: The positive expression rate of CD68 in LSCC tissue was 82% (37/45), which was higher than the 10% (2/20) expression rate of the peritumoral tissue (P<0.05). The CD34-MVD positive expression rate in the LSCC tissue was 26.5±6.4, which obviously higher than 12.2±4.0 expression rate of the peritumoral tissue (P<0.05). The positive expression rates of both CD68 and CD34-MVD were higher in the lymph node metastasis (LNM) positive group than in the LNM negative group. The expression of CD68 had positive correlation with CD34-MVD. The 5-year disease-free survival rate in the group with the low CD68 expression was significantly higher than that in the group with high CD68 expression (76% vs. 42%, respectively). CONCLUSION: The high expression of CD68-TAMs in LSCC and its positive correlation with CD34-MVD illustrates that both play an important role in promoting the metastasis and angiogenesis of this cancer. Their expression was also positively correlated with the prognoses of these patients, suggesting that they could be used as important prognostic markers for LSCC.

OBJECTIVE@#To study the expression of CD68 antibody marked tumor associated macrophage TAMs and matrix solution element MMP-7 in laryngeal squamous carcinoma tissue and the relationship with clinicopathological parameters, so that to explore the relationship between the expression of the two molecular markers and laryngeal cancer tissue microvascular density (MVD).@*METHOD@#Immunohistochemical method was employed to detect the expression of CD68 and MMP-7 in 65 cases (laryngeal squamous carcinoma tissue in 45 cases; peritumoral nontumor tissue in 20 cases) and CD 34 antibody marked MVD expression.@*RESULT@#CD68 positive rate in squamous carcinoma tissue (82.2%, 37/45) is obviously higher than that in the peritumoral tissue (15%, 3/20) (P < 0.05), and MMP-7 positive rate in squamous carcinoma tissue is significantly different from that in peritumoral tissue (71.1%; 25%) (P < 0.05). The expression rate of CD34-MVD in laryngeal squamous carcinoma tissue( 26.52 +/- 6.36 )is higher than that in peritumoral tissue (12.23 +/- 4.01) (P < 0.05). In lymph node metastasis group, the positive expression rates of CD68 and MMP-7 are higher than those in the group without lymph node metastasis. MMP-7 showed no correlation with cancer stage, and CD68 was related with cancer stage; CD68, MMP-7 and CD34- MVD have positive correlation.@*CONCLUSION@#The high level of expression of TAMs and MMP-7 in laryngeal cancer tissue and the positive correlation with MVD illustrate that both of the markers play important roles in promoting laryngeal squamous carcinoma tissue metastasis and angiogenesis, which can be used as important markers to evaluate the invasion and metastasis of laryngeal cancer.

ABSTRACT

Objective To explore the effect of quality control circle (QCC) on health education in stroke rehabilitation nursing. Methods Such activities were conducted in the VIP ward of neurological rehabilitation in our hospital as:theme selecting, activity planning, status mastering, cause analyzing, countermeasures formulating and implementing, etc., in accordance with QCC theory from January to April, 2014. Results The satisfaction of stroke patients to health education rose from 80.1%to 91.1%(P<0.001), and the dissatisfaction to guid-ance of activities of daily living and prevention of complications decreased significantly (P<0.05) after the implementation of QCC pro-gram. All the circle members benefited from the improvement of QCC technique, team work, professional knowledge, communication and coordination, activity confidence, duty and honor. Conclusion Carrying out the QCC activities can play a positive role on health education in stroke rehabilitation nursing.

Objective To discuss the application of scenario simulation technique in emergency rescue training of hospital grave pubic health events.Methods Based on the simulate function in the scenario simulation technique,we designed the emergency drill script of hospital grave pubic health events,set the simulate scene,communication system and indicators measuring emergency rescue,drilled and trained the hospital rescue crews,compared with the rescue crews trained in routine method.40 nurses were selected before or after the application of scenario simulation technique separately.Results The qualified rate of indicators such as communication and medical rescue system that were trained by scenario simulation method was higher than the rescue crews trained in routine method.Conclusions Scenario simulation technique would improve the training effect of emergency rescue in hospital grave pubic health events.

ABSTRACT

<p><b>OBJECTIVE</b>To study the change of synaptic onset latency and threshold in primary auditory cortex (A1) during the development of SD rat.</p><p><b>METHODS</b>Extracellular recording was used to locate A1, followed by transferred to loose-patch and whole-cell patch in vivo to record the spike activity, synaptic onset latency and threshold responses respectively. Rats were divided into 4 groups according to ages, postnatal 12-15 days, 16-18 days, 19-24 days and adult (> 3 months).</p><p><b>RESULTS</b>1. The onset latency of local field potential in A1 of adult rats[(10-20)ms] was shorter than young rats[(20-30)ms]. 2. During development, the onset latency of spikes of a single neuron in response to white noise pulses decreased. And the latency in young rats P12-15 [(40.15 ± 2.67) ms] and P16-18 [(33.86 ± 4.61) ms] were longer than in adults [(22.93 ± 2.94) ms] (ANOVA-test, t = 4.330 and 1.995, P = 0.00 and 0.04) . However, the onset latencies of P19-24 [(24.80 ± 3.63) ms] and adult had no significant difference (P > 0.05). 3.Synaptic onset latencies of both excitation and inhibition were significantly longer in P12-15[ (38.94 ± 1.90) ms, (35.26 ± 2.40) ms] and P16-18[ (32.68 ± 2.52) ms, (30.24 ± 2.18) ms] than in adults [(19.4 ± 1.06) ms, (18.91 ± 0.77) ms] excitation (t = 6.255 and 4.662, P < 0.01) inhibition (t = 8.918 and 4.820, P < 0.01) showed significant difference. Whereas the onset latencies of P19-24[ (23.67 ± 2.46) ms, (21.43 ± 1.80) ms] and adults displayed no prominent difference(P > 0.01). Meanwhile, the difference between the onset latencies of excitation and inhibition became narrower during development[ (3.15 ± 1.02) ms, (2.01 ± 0.73) ms, (1.79 ± 0.85) ms, (0.39 ± 0.48) ms]. P12-15 had notably difference in comparison to adults (t = 1.739, P < 0.01). 4. The thresholds of synaptic response were notably higher in P12-15 (40.0 ± 1.6) dB and P16-18 (41.3 ± 11.6) dB when compared with adults (30.9 ± 0.6) dB (t = 5.284 and 5.867, P < 0.01) . While that of P19-24 (35.0 ± 32.7) dB showed no distinct difference (P > 0.01).</p><p><b>CONCLUSION</b>Single neuron spiking activity, synaptic onset latency and threshold evoked by sound stimulus gradually mature during the development in rat A1.</p>

ABSTRACT

@#Objective To explore the effect of quality control circle (QCC) on health education in stroke rehabilitation nursing. Methods Such activities were conducted in the VIP ward of neurological rehabilitation in our hospital as: theme selecting, activity planning, status mastering, cause analyzing, countermeasures formulating and implementing, etc., in accordance with QCC theory from January to April, 2014. Results The satisfaction of stroke patients to health education rose from 80.1% to 91.1% (P<0.001), and the dissatisfaction to guidance of activities of daily living and prevention of complications decreased significantly (P<0.05) after the implementation of QCC program. All the circle members benefited from the improvement of QCC technique, team work, professional knowledge, communication and coordination, activity confidence, duty and honor. Conclusion Carrying out the QCC activities can play a positive role on health education in stroke rehabilitation nursing.

ABSTRACT

Objective To study the prevalence of metabolic syndrome (MS) within the Uyghur people with abnormal glucose metabolism in Xinjiang province according to three different diagnostic criteria and effects of accumulation of MS components on blood lipid profile.Methods Components of MS and blood lipid profile were observed with 666 Uyghur people with abnormal glucose metabolism after cluster randomized sampling method for selecting in Xinjiang province,and analyzed with the recommended diagnostic criteria by the National Cholesterol Education Program-Adult treatment Panel Ⅲ(NCEP-ATP Ⅲ,2001),the Chinese Diabetes Society under Chinese Medical Association (CDS,2004) and the International Diabetes Federation (IDF,2005).Results Prevalence of MS was 21.92% (14.67%),23.72% (15.88%),57.51% (38.48%) with the criteria by NCEP-ATPⅢ,CDS,and IDF,respectively.IDF was the best among three criteria.The most combination was the abnormal glucose metabolism,dyslipidemia and obesity.In the ATPⅢ (2001),with the increase of number of components of metabolic syndrome,highdensity lipoprotein cholesterol (HDL-C) drop,low density lipoprotein cholesterol (LDL-C) increased; there was not significant difference in the CDS.Conclusions The incidence of MS was high in the Uyghur people with abnormal glucose metabolism in Xinjiang province.Most of them complicated with dyslipidemia.One of common components of MS included the elevated triglycerides,decreased HDL-C,and increased size of LDL-C particles,which could easily cause cardiovascular disease.

ABSTRACT

A total of 60 patients with an acute attack of asthma were studied.On presentation,fractional exhaled nitric oxide (FeNO) and the serum concentrations of procalcitonin (PCT) and C-reactive protein (CRP) were measured.And sputum culture was also performed.The patients were re-evaluated while returning to their clinical remission states.They were classified into 2 groups:patients with bacterial infection (group A) and those with nonbacterial infection (group B).The levels of FeNO were significantly higher in patients with acute exacerbation than those in remission.No difference existed between groups A and B ( P > 0.05 ).The levels of PCT and CRP of group A with acute exacerbation were significantly higher than those of group B( all P <0.05).While in remission,the levels of PCT and CRP decreased significantly in group A ( P < 0.05 ) ; But compared with exacerbation,the levels of PCT and CRP showed no change in group B (P >0.05).And no differences existed between two groups while in remission (P > 0.05 ).An elevation of FeNO indicates the acute exacerbation of asthma.And the increased serum levels of PCT and CRP are associated with bacterial infection.
